OUTPATIENT GUIDEHOW TO CHECK-IN
INITIAL (FIRST) VISIT
Fill out and submit ‘Application Form for New Patient / Registered Information Change Notification and Consent Form’ with your health insurance card to INFORMATION (Sogo Annai 総合案内) for New Patient. This is to issue your hospital ID card and create medical record. If you arrive before 8:00 a.m., obtain numbered ticket at INFORMATION.

FORGOT TO BRING YOUR HOSPITAL ID CARD
Go to Window 1 – Return visit. (We wil check-in after confirming your name and date of birth)
*Night, Holidays, Saturday afternoon: Go to Window 3 – Accounting / After-Hours Reception -
WITH YOUR HOSPITAL ID CARD
and have an appointment or visited our hospital within 3 monthsCheck-in by yourself with Reception Machines.

but did NOT visit our hospital …a) more than 3 months to 1 year
Go to Window 1 – Return visitb) more than 1 year
Fill out ‘Application Form for New Patient / Registered Information Change Notification and Consent Form’ then submit it to Window 1 – Return visit.

On the day of admission, please follow the designated procedures such as filling out applications at the Inpatient Lounge. -

Please bring your health insurance card and other cards as needed for your admission. -

Please check the time printed on the appointment sheet for your scheduled admission.
